For borrowers: By submitting this form, you hereby consent to have your information compiled by CHFA for the purpose of participating in its future marketing and communication opportunities (collectively, the "Marketing Campaign") and authorize CHFA to use such information to contact you in the future for such purposes. CHFA in no way guarantees that you will be contacted or, if contacted, that your story will be used in any current or future Marketing Campaign.
For participating lenders: By submitting this form and providing borrower contact information, you affirm that you have obtained the borrower’s prior written consent to use such borrower’s contact information in the manner set forth herein. Further, you agree that CHFA may contact you in regard to the Marketing Campaign.
Revocation of consent: This consent is effective immediately upon submission and shall remain valid until revoked. If you choose to revoke this consent or request to have your personal data deleted, you must submit a request in writing. You can submit a request to withdraw this consent by providing written notification to CHFA at privacy@chfainfo.com, or by the united postal service or similar mail-carrier service with the letter addressed to Colorado Housing and Finance Authority, 1981 Blake Street, Denver, Colorado, 80202, Attn: Legal Risk Management. When submitting a request to withdraw this consent, please outline the details of your request. Such written requests will be effective ten (10) business days after CHFA has received the request. Please note revocation, as provided in this paragraph, will only be effective to remove your personal data for future use by CHFA, and will not remove your information from any Marketing Campaign that has already been published.
In the event CHFA contacts you to participate in a Marketing Campaign, you understand that CHFA will require you to execute a formal consent and release.
